In addition to the core services, the Premise Health medical staff at the ÃÛ¶¹ÊÓƵ Health Center is equipped to administer a COVID-19 Antigen (Sofia 2) test** on-site, providing results in 20 minutes.
*Visits will incur a charge at the time of service until your deductible is met. Once the deductible is met, the ÃÛ¶¹ÊÓƵ Group Plan pays 100% of the primary care office visit.
** There is no cost to the employee for the COVID-19 Antigen Sofia 2 test.
Connect one-on-one with an onsite master's-level certified and licensed counselor at the ÃÛ¶¹ÊÓƵ Health Centers. Virtual appointments are also available, depending on state licensing. Visits will incur a charge of $65 - $100 (depending on location) at the time of service until your deductible is met. Once the deductible is met, the plan covers any visits as an in-network service.
